Import survey data from excel file as case classifications. Multiple choice options listed as individual columns in excel

Hello!  I am trying to import an excel file with survey responses.  Currently the excel spreadsheet is set up so that each choice for a multiple choice question has its own column as in screen shot 3.  This results in crazy case classifications so that "Alberta", "Ontario", and "Quebec" are listed as individual case classifications rather than as values within the case classification "Question 1: Do you live in"   

I want the multiple choice options to be listed as *values* under the case classification rather than as case classifications in and of themselves.  Any advice?

Sorry, to clarify, right now, my multiple choice options on the survey are showing up in Nvivo as individual case classifications themselves rather than as values OF case classifications.  Any ideas on how to fix this?
